To provide you with the highest standard of care and make the most of your appointment time, we kindly ask all patients to:

• Complete the Medical History & Medication Form:
  After booking, you’ll receive a confirmation email with a link to our medical history and medication form. Please fill this out before your visit. This allows our clinicians to focus on your main concerns, rather than paperwork, during your appointment.

• Send a Medical Summary from Your GP:
  Please request a short medical summary from your GP and email it to us prior to your appointment. This helps us understand your medical background and ensures your treatment is safe and tailored to your needs.
